San Angelo Man Sues Meadow Creek Over Wife’s Death

 

SAN ANGELO, TX — The husband of a Tom Green County woman who died in April 2023 has filed a lawsuit against the operators of Meadow Creek Nursing and Rehabilitation, alleging negligent and grossly negligent care caused her to develop pressure wounds, severe malnutrition, dehydration and significant weight loss. 

Terry Williams, individually and on behalf of the estate of his wife, Deborah Williams, sued Caraday Meadow Creek LLC d/b/a Meadow Creek Nursing and Rehabilitation, Caraday Healthcare TX3 LLC and Caraday Healthcare LLC in Tom Green County District Court. The suit, cause No. B230494C, was filed in the 119th Judicial District Court. 

According to the petition, Deborah Williams was admitted to the facility on Jan. 21, 2023. Over a 36-day period from Jan. 28 to March 5, 2023, she lost 17 pounds, dropping from 152 pounds to 135 pounds, the lawsuit states. Court documents allege she suffered dehydration, malnutrition, weight loss and pressure ulcers while in the defendants’ care. 

On March 9, 2023, staff noted labored breathing and an oxygen saturation level of 82% and called 911 to transport her to Shannon Medical Center. She died there on April 5, 2023. Her death certificate listed metabolic encephalopathy and acute kidney failure as the cause of death, with dementia, diabetes, hypertension and coronary artery disease as significant contributing conditions, the suit says. 

The lawsuit claims the defendants knew of Deborah Williams’ pre-existing conditions, including Alzheimer’s disease and type 2 diabetes, but failed to provide adequate monitoring, staffing, training and care. It alleges violations of state and federal nursing home regulations regarding staffing levels, care plans, clinical records and resident safety. The petition further accuses the operators of corporate negligence through undercapitalization, chronic understaffing based on resident acuity levels and prioritizing profits from Medicare and Medicaid funding over adequate care. It seeks economic and non-economic damages, including punitive damages for gross negligence. 

Each of the three defendants filed original answers in January 2024. In separate filings, Caraday Healthcare LLC, Caraday Meadow Creek LLC d/b/a Meadow Creek Nursing and Rehabilitation, and Caraday Healthcare TX3 LLC generally denied all allegations in the petition and asked the court to render judgment in their favor with costs. 

The case is set for a dismissal docket on June 10, 2026, at 1 p.m. before Judge Gonzalo Rios.

Sad that people die like this, nursing homes have long been known to be understaffed all the time but this sounds like what happens to most people when they get put in nursing home, her and her body gave up and shutdown.
